When you choose one of the forms listed in the menu, you will be downloading that form onto your computer.

These forms are created with Microsoft Word 2000. Most of them are templates. If you do not have Microsoft Word on your computer you can install the FREE Wordviewer 2007 version (PC only), Or download FREE Open Office (PC & MAC).

  • Click on the form you need to start the download. Once the form opens, go to the next step.
  • From the "File" menu, choose "Save As" and save the form to a place on your computer.
  • Click in one of the fields and start entering data. To move to the next field you just hit the "Tab" key.
  • When you are done with all of your entries, save the form.
  • Attach a copy of the form to an e-mail to your adviser.

Student Forms

2013/14 1st Semester Grade Report Form

Semester reports are used to document educational progress each semester. Include information in these reports to document educational history, progress and achievement levels. This report is due on 12/18/2013. Click on the link to the left to download a copy. Fill the form out and submit it to your advisor ASAP if you have not done so.

Middle School Co-Curricular Participation Consent Form

A reminder for parents of students who participate in sports at a local school. The Connections principal, Lee Young, must sign the Co-Curricular Participation Form located in the packet from the coaches. Eligibility is required by KPSAA rules. The dates for various sports are listed at the bottom of the form.

Eligibility Grade Report Form (Middle School)

Eligibility Reports are due every Monday before 12:00 pm for the duration of the student’s participation of an activity. This is the form for Middle School students.

High School Course Contract Form

This form is used for parents to create classes using non-textbook type materials and lessons. Examples would include practical arts classes such as welding or foods where the student will work with someone completing a minimum of 67.5 hours of instruction and practice. The form should include what activities and skills will be included in the instruction and how their progress will be assessed.

High School Planner Form

This form is for laying out your complete High School plan. It is an excellent way to keep track of your progress throughout your High School years.

Individual Learning Plan (ILP) Form

The ILP is the individualized educational program parents design for their children. The ILP outlines goals and objectives, and lists instructional material and methods to be used throughout the school year. All reimbursements are based on ILP content. Fine Arts, P.E., Instructor Reporting Form

This is the form that the instructor or parent fills out for reporting a students participation in fine arts, physical education, or a tutored class. Click on the link to the left to download a copy.

Log Form For Fine Arts, P.E.

This is the form you will use to keep track of a students activity for physical education or fine arts. Click on the link to the left to download a copy.

OJT/Work Co-op Log Form

This is the form you will use to keep track of a students On Job Training or Work Co-op activity. Click on the link to the left to download a copy.

Video/Picture Persmission Form

Please note this is a 2 page form.

Classroom activities and events sponsored by school occasionally are photographed or videotaped by school personnel, students, or representatives of the news media for publication in newspapers or broadcast on television or the Internet. If for any reason you do not wish to have your child(ren) photographed or videotaped for these purposes, please download the form, fill it out and sign it, and return this form to school within five (5) school days.


 




Miscellaneous Forms

Expenditures Pre Approval Form

This form is for pre-authorized approval of educational expenses. This form is due 2 weeks prior to the date of the expenditure. After purchases are made, a Reimbursement Form must be submitted with original receipts attached.

This form is used to make a Curriculum order. Click on the link to the left to download a copy. Fill the form out and submit it to your students advisor.

Instructor and Tutor Application Form

W-9 Form

If you would like to become a tutor or instructor to teach students, you will need to submit one of these forms for approval.

It also requires a Borough Sales Tax registration and W-9 form. Click on the link to the left to download a copy. The W9 is required to be turned in with the application.

KPC Payment Authorization Form

Payment Authorization Form KPC, University of AK, Soldotna/Homer. Payment will not be authorized until Connections has received verifying documentation from the University (a receipt). Click on the link to the left to download a copy.

Parent Reimbursement Form

This form must be filled out when submitting a request for reimbursement. Please attach all original receipts. If the receipt does not list items, please identify specific titles on back. Connections will reimburse academic curriculum upon receipt. Fine Arts, PE, and tutoring costs will be reimbursed upon completion of services. Please allow a minimum of 2 weeks for payment reimbursement. You must also have this form submitted to get reimbursement: Enrollment W-9 Form

Students 18 and older Disclosure Form.

Under the Family Educational Rights and Privacy Act (FERPA), the Kenai Peninsula Borough School District (KPBSD) is permitted to disclose information from the education records of a student age 18 and older to your parents, if your parents (or one of your parents) claim you as a dependent for federal tax purposes.

Computer/Laptop Usage & Technical Support Available

Connections provides students with a windows-based computer. Families have the option of checking out a desktop computer or laptop or joining the Rent to Own Program. Technical support is also available to help with computer hardware and software problems.
